Fatigue on the Wegovy pill
It is on the label. Fatigue was reported by 11 percent of people on the injection against 5 percent on placebo, and the tablet trial found the same types and frequencies of effects. On the forums it is the second most discussed effect after nausea, and the one that surprises people most, because nobody warned them.

The short answer
It is on the label. Fatigue was reported by 11 percent of people on the injection against 5 percent on placebo, and the tablet trial found the same types and frequencies of effects. On the forums it is the second most discussed effect after nausea, and the one that surprises people most, because nobody warned them.
What the label says
Fatigue sits in the middle of the label's side effect table, between headache and dyspepsia.
From the labelFatigue: 11 percent with WEGOVY 2.4 mg injection, 5 percent with placebo.
The footnote says the line includes asthenia, the medical word for weakness. The tablet trial, reported a few paragraphs later, found effects similar in type and frequency to that table. So the label's number for the tablet is the injection's: about one person in nine, against one in twenty without the drug.
What the label does not say
It does not say when fatigue starts, how long it lasts, or which step brings it. It does not offer a remedy. Its one tool for any step that is hard to tolerate is to delay the escalation, and that tool belongs to the prescriber.
What people actually do
On the forums, fatigue is the second most written-about effect, after nausea, and the one that surprises people most. The words are strong: could sleep twenty hours, sick of being tired, no energy for the gym or for cooking. It is described most on 4 mg, sometimes as a flatness of mood as much as of the body, and it pushes people to step back down or to stop as often as nausea does.
What people find useful is separating the fatigue that is the pill from the fatigue that is not eating: the appetite goes, the meals shrink, the water is forgotten, and the afternoon collapses. The Medication Guide's warnings about dehydration point the same way, and so does the low blood sugar warning for people with type 2 diabetes.
Oolite's journal has a line for how the day felt, two taps, kept by step. A week of heavy days is a line on a chart at the next appointment, and the decision about the step is the prescriber's.
Questions people ask
Is fatigue worse at a particular step?
The label does not report effects by step for the semaglutide tablet. On the forums, 4 mg is the step where fatigue is described most, alongside nausea and reflux. That is description, not a rate.
Does it go away?
The label does not say. It writes the escalation to reduce gastrointestinal effects and says nothing about the course of fatigue. People describe it easing over weeks on a step and returning at the next.
Could the fatigue be something else?
It could be dehydration, which the Medication Guide warns about when nausea, vomiting or diarrhea are present, or low blood sugar in people with type 2 diabetes on insulin or a sulfonylurea. Both are reasons to call. So is fatigue that is new, heavy and does not lift.
Should I stop the pill because of it?
That is a prescriber decision. The label's tool for a step that is not tolerated is to delay the escalation, not to stop, and the prescriber can weigh fatigue against everything else.
