Levothyroxine and the Wegovy pill: in what order?

Two sentences from two sections. The first is the morning rule.

Why levothyroxine is the one named
Both tablets want an empty stomach, so they compete for the same half hour, and the manufacturer tested what happens when they share it. The answer was more levothyroxine in the blood, not less: a third more. For a hormone that is dosed in micrograms and checked by blood test, that is enough for the label to ask for monitoring.
The label does not say to stop either one, and it does not give an order beyond the 30 minutes. It leaves the arrangement to the prescriber, who can move levothyroxine to another time of day or adjust it after a blood test.
What people actually do
On the forums the thyroid question is the most common one about other medications. People describe a routine of three steps: the Wegovy pill at the first alarm with a sip of water, the timer, then the rest of the morning's tablets with breakfast. Some have moved levothyroxine to bedtime on their prescriber's advice. Several mention a thyroid test a few weeks after starting.
The same three step routine covers the blood pressure pill, the antidepressant, the reflux tablet and the vitamins: none of them goes in the first 30 minutes.

Questions people ask
Levothyroxine also has to be taken on an empty stomach. Which one goes first?
The Wegovy pill label says to take it first thing, then to wait at least 30 minutes before other oral medications. Levothyroxine's own timing rules are between you and your prescriber, who can also move it to another time of day.
What does the 33 percent mean for me?
It means the same levothyroxine tablet delivered a third more into the blood when taken with the semaglutide tablet in the manufacturer's study. The label asks for monitoring. In practice that is a thyroid blood test and a possible dose adjustment, decided by your prescriber.
Does the same apply to blood pressure pills, antidepressants, supplements?
The 30 minute rule applies to every oral medication and to supplements, because the label says other oral medications and eating food. The 33 percent figure is specific to levothyroxine. For other drugs the label speaks of delayed stomach emptying and asks for monitoring where the margin is narrow.
Can I take my other pills with the sip of water at the same time?
No. The label says water only, up to 4 ounces, and other oral medications after at least 30 minutes.
