The Wegovy pill and pregnancy: the two-month rule

The label says to stop the pill at least two months before a planned pregnancy, because of semaglutide's long half-life. It also says that breastfeeding is not recommended on the tablets, because the tablet's helper compound passes into milk. Both are conversations to have with the prescriber before, not after.

The short answer

The label says to stop the pill at least two months before a planned pregnancy, because of semaglutide's long half-life. It also says that breastfeeding is not recommended on the tablets, because the tablet's helper compound passes into milk. Both are conversations to have with the prescriber before, not after.

What the label says

Three passages, in the section on specific populations.

On planning a pregnancy:

From the label

Discontinue WEGOVY in patients at least 2 months before they plan to become pregnant to account for the long half-life of semaglutide.

On pregnancy itself, the label says that data from clinical trials and pharmacovigilance are insufficient to establish the risk, that animal studies suggest there may be a risk to the fetus, and that a pregnancy exposure registry exists. The registry's number is 1-877-390-2760.

On breastfeeding, the tablet has its own paragraph:

From the label

Data from a clinical lactation study with semaglutide oral tablet formulation reported semaglutide concentrations below the lower limit of quantification in human milk. However, SNAC and/or its metabolites are present in human milk.

And the conclusion the label draws from it: breastfeeding is not recommended during treatment with the tablets.

Why the tablet and the injection differ here

The tablet carries SNAC, the compound that lets semaglutide survive the stomach. The injection does not. SNAC is what the lactation study found in milk, and the label notes that the enzymes that clear it may be less active in infants. That is the whole reason the tablet's breastfeeding line is stricter than the injection's.

What people actually do

The two month rule surprises people who assumed a daily tablet could be stopped the day before. The half-life is the answer, and it is the same one behind every other slow thing about this pill: it takes weeks to build and weeks to leave.

The question the forums cannot answer, and should not, is the one about an unplanned pregnancy discovered mid-treatment. The label's answer is the registry and the prescriber. This page has no other.

Oolite records the last tablet and draws the level falling over the following weeks. The decisions around it are the prescriber's.

Questions people ask

Why two months?

The label ties it to the half-life. Semaglutide takes about a week to fall by half and is still present five to seven weeks after the last tablet. Two months is the label's margin for it to be gone.

I found out I am pregnant while on the pill. What now?

The label says the available data are insufficient to say what the risk is, and it points to a pregnancy registry run by the manufacturer, which your prescriber can enrol you in. The first call is to the prescriber, today.

Can I breastfeed on the Wegovy pill?

The label says breastfeeding is not recommended during treatment with the tablets. Semaglutide itself was below the measurable level in milk, but the tablet's absorption helper, SNAC, was present, and infants may clear it more slowly.

Is the rule different for the injection?

The two month rule before pregnancy is the same for both. The breastfeeding line is specific to the tablet, because the injection has no SNAC.
