The Wegovy pill with type 2 diabetes

The label has two specific things to say. Blood concentrations on the tablet were lower and more variable in people with type 2 diabetes, so some may not reach a working level, and an inadequate response on the tablet is a reason to consider the injection. And the risk of low blood sugar is higher with insulin or a sulfonylurea: 6 percent against 2 in the trial.

The short answer

The label has two specific things to say. Blood concentrations on the tablet were lower and more variable in people with type 2 diabetes, so some may not reach a working level, and an inadequate response on the tablet is a reason to consider the injection. And the risk of low blood sugar is higher with insulin or a sulfonylurea: 6 percent against 2 in the trial.

What the label says

The pharmacology section has a paragraph written only for this group.

From the label

The lower semaglutide blood concentrations after administration of WEGOVY tablets in patients with type 2 diabetes may be associated with reductions in absolute bioavailability. Given the differences in average semaglutide blood concentrations after administration of WEGOVY tablets and WEGOVY injection, and the higher variability in semaglutide blood concentrations after administration of WEGOVY tablets in all patient populations, some patients with type 2 diabetes taking WEGOVY tablets may experience subtherapeutic semaglutide blood concentrations. In patients with type 2 diabetes with an inadequate response to WEGOVY tablets, consider alternative therapies, including switching to WEGOVY injection.

Three facts in one paragraph: the tablet gave lower average levels in people with type 2 diabetes; the tablet varies more in everyone; and if the tablet is not working for someone with type 2 diabetes, the label points at the injection.

The other number

The side effect table has a line just for this group: low blood sugar in 6 percent of people with type 2 diabetes on the injection against 2 percent on placebo, among those not on insulin. The warning section says the risk is higher with insulin or a sulfonylurea and asks prescribers to consider lowering those doses. The Medication Guide asks patients to check their blood sugar before starting and during treatment, and lists the signs of a low.

What people actually do

People with type 2 diabetes on the Wegovy pill describe two conversations with their prescriber. The first, at the start, is about the insulin or the sulfonylurea, and the label's warning is the reason for it. The second, months in, is about numbers that have not moved, and the label's paragraph on inadequate response is the reason that conversation can end with a switch rather than a shrug.

The morning rules matter here as much as anywhere, because the label's concern is precisely how little of the tablet gets in.

Oolite keeps the mornings and the steps. It does not read glucose. The meter and the prescriber do that, and the label's paragraph is worth bringing to them.

Questions people ask

Why would the tablet work less well with type 2 diabetes?

The label says average concentrations on the 25 mg tablet were lower in people with type 2 diabetes than with the injection, possibly because less of the tablet is absorbed, and that with the tablet's higher variability some may end up below a working level. It does not say why absorption differs.

What does the label suggest if it is not working?

In its words: in patients with type 2 diabetes with an inadequate response to the tablets, consider alternative therapies, including switching to the injection.

How common is low blood sugar?

In the trial of people with type 2 diabetes not on insulin, 6 percent on the injection had glucose below 54 mg/dL against 2 percent on placebo. The label says the risk is higher with insulin or a sulfonylurea and asks prescribers to consider reducing those.

Is Ozempic tablets the same thing?

Same molecule, different product: Ozempic tablets are approved for type 2 diabetes at 1.5, 4 and 9 mg. The Wegovy pill is approved for weight management and cardiovascular risk reduction at up to 25 mg. Your prescriber chooses between them.
