Missed doses

What if I missed a dose of Foundayo?

Take it as soon as you remember, and do not double the next one. That is the label's rule, and it is the opposite of the semaglutide pills. If you miss seven or more days in a row, the label says to restart the dose escalation at a lower strength.

The short answer

Take it as soon as you remember, and do not double the next one. That is the label's rule, and it is the opposite of the semaglutide pills. If you miss seven or more days in a row, the label says to restart the dose escalation at a lower strength.

What the label says

From the label

If a dose is missed, instruct patients to take the dose as soon as possible. Advise patients not to double up the next dose. If 7 or more consecutive doses are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions.

Three rules in one paragraph. Take the missed tablet when you remember. Never two in a day. And after a week or more without any, do not go straight back to the strength you were on.

"As soon as possible", not "skip it"

The semaglutide tablets say the opposite: skip the missed dose, take the next one tomorrow. Neither label says why. What differs is speed. Orforglipron, the molecule in Foundayo, has an elimination half-life of about 29 to 49 hours. A day without it takes a real bite out of the level, and a tablet taken late in the day still restores most of it. Semaglutide, with a one week half-life, loses about 9 percent of the modelled level in a day.

What one day does to the level

In a model built on the label's half-life (1.6 days), one day without a tablet lowers the level by about a third. Two days, by more than half. After a week without tablets the model is below 5 percent, and the label treats a return as a new start, at a lower strength, to reduce the risk of stomach side effects.

None of this is a reason to panic over one missed morning. It is the reason to take it when you remember rather than wait for tomorrow.

What people actually do

Foundayo is newer and its forums are thinner than the semaglutide ones, and the missed-dose question comes up mostly from people who came from the Wegovy pill and kept its habit of skipping. The label's answer is different for this tablet, and the habit is worth unlearning.

The other pattern is the travel week: a bottle left at home, a week off, then a straight return to the old strength. That is exactly the situation the seven day rule is written for, and it belongs to the prescriber.

Questions people ask

I remembered in the evening. Do I take it then?

The label says as soon as possible. Foundayo has no morning rule and no food rule, so an evening dose is what the label describes for a missed morning one.

Can I take two tomorrow to catch up?

No. The label says not to double up the next dose, and separately that you should not take more than one tablet per day.

I missed a whole week. Do I go back to 0.8 mg?

The label says that after seven or more consecutive missed doses, the dose escalation is restarted at a lower dosage to reduce the risk of stomach side effects. Which strength that is, is your prescriber's call.

Why is the rule different from the Wegovy pill?

The labels do not say why. They are different molecules at different speeds: orforglipron has a half-life of 29 to 49 hours, so a missed day costs about a third of the modelled level; semaglutide has a one week half-life, and its label says to skip the missed dose.
