Two small tablet bottles on a kitchen table, one older, a woman's hand moving the newer one to the front, a glass of water beside them

Switching

I still have Rybelsus. How do I switch to Ozempic tablets?

The label has the table. After the first 30 days on either tablet, 7 mg of Rybelsus becomes 4 mg of Ozempic tablets and 14 mg becomes 9 mg, starting the new tablet the day after the last old one. During the first 30 days, the label says not to switch at all. The two are the same molecule and are not interchangeable milligram for milligram.

The short answer

The label has the table. After the first 30 days on either tablet, 7 mg of Rybelsus becomes 4 mg of Ozempic tablets and 14 mg becomes 9 mg, starting the new tablet the day after the last old one. During the first 30 days, the label says not to switch at all. The two are the same molecule and are not interchangeable milligram for milligram.

What the label says

The Rybelsus label was rewritten in January 2026 to cover both tablets, and it has a switching section.

From the label

Do not switch between RYBELSUS and OZEMPIC tablets during the initiation phase (Days 1 to 30). After 30 days of RYBELSUS or OZEMPIC tablet treatment, patients may switch between RYBELSUS and OZEMPIC tablet products. When switching between RYBELSUS and OZEMPIC tablets, initiate the other semaglutide tablet product the day after discontinuing the previous semaglutide tablet product.

And the table that goes with it:

RybelsusOzempic tablets
7 mg once daily4 mg once daily
14 mg once daily9 mg once daily

The starting strengths, 3 mg and 1.5 mg, are not in the table because the label does not want a switch during that first month.

Why not during the first 30 days

The label calls the first month the initiation phase and says the starting dose is not effective for glycemic control: it is there for the stomach. Switching tablets mid-initiation would mix two starting doses that the label counts separately, so it asks you to finish the first month on whichever tablet you began.

Not substitutable milligram for milligram

The two tablets contain the same semaglutide in two formulations, and the newer one delivers the same exposure with fewer milligrams. That is why the label says they are not substitutable on a mg-to-mg basis: 9 mg of Ozempic tablets is not a smaller dose than 14 mg of Rybelsus, it is the same dose under a new number.

What people actually do

Since Rybelsus left US pharmacies in May 2026, the practical question is the bottle in the cupboard. People describe finishing it and picking up Ozempic tablets at the next refill, which the label allows after the first month, with the switch happening overnight. The confusion is the number: a pharmacist handing over 4 mg to someone used to 7 mg looks like a cut, and it is not.

Oolite accepts Rybelsus in onboarding, maps it to the Ozempic tablets strengths on the label's table, and keeps the morning rules the same on both.

Questions people ask

Can I just finish the Rybelsus bottle first?

The label allows either tablet after the first 30 days, and the switch happens the day after the last tablet of the old one. Whether to finish the bottle or switch now is a question for your pharmacist, who also knows what your pharmacy can still dispense.

I am in my first month on Rybelsus 3 mg. Do I switch to 1.5 mg?

The label says not to switch between the two tablets during the initiation phase, days 1 to 30. Finish the first month on the tablet you started, then switch on the schedule your prescriber sets.

Is 9 mg of Ozempic tablets a lower dose than 14 mg of Rybelsus?

It is the same exposure under a smaller number. The label puts the two on the same row of its switching table and says they are not substitutable milligram for milligram, which is precisely because the numbers do not mean the same thing.

Do the morning rules change?

No. Both labels say empty stomach, up to 4 ounces of water, no other liquids, then at least 30 minutes.
