An injection pen and a small tablet bottle on a kitchen counter with a glucose meter, a calendar behind them, morning light

Switching

Switching from the Ozempic injection to Ozempic tablets

The label writes it for one case: people on the 0.5 mg weekly injection. One week after the last injection, start 4 mg or 9 mg of Ozempic tablets once daily. The label does not give a table for other injection doses, and it is the prescriber who picks 4 or 9.

The short answer

The label writes it for one case: people on the 0.5 mg weekly injection. One week after the last injection, start 4 mg or 9 mg of Ozempic tablets once daily. The label does not give a table for other injection doses, and it is the prescriber who picks 4 or 9.

What the label says

From the label

Switching from OZEMPIC Injection to OZEMPIC Tablets. Patients taking the 0.5 mg dose of OZEMPIC injection may switch to OZEMPIC tablets. One week after discontinuing 0.5 mg of subcutaneous OZEMPIC injection, start 4 mg or 9 mg of OZEMPIC tablets orally once daily.

One injection dose, one waiting period, two possible starting strengths. The label does not write a rule for the 1 mg or 2 mg injection, and it does not say which of 4 or 9 mg a given person should take; that is the prescriber reading the glucose numbers.

What changes on the day of the switch

Everything about the morning. The injection asked for nothing: any day, any time, with or without food. The tablet asks for an empty stomach on waking, a sip of water up to 4 ounces and nothing else, the tablet swallowed whole, and 30 minutes before food, drink or other oral medications. A missed tablet is skipped, not doubled.

The one week wait is the same margin the Wegovy label gives for its own switch. Neither label says why.

What people actually do

People moving from the pen to the tablet do it for the needle, for travel, or because the tablet was what the pharmacy had. What they describe learning is the morning, and the coffee. What surprises them is that the tablet does not start at the bottom: the label sends someone from the 0.5 mg injection straight to 4 or 9 mg.

The question the label does not answer, the higher injection doses, is the one people ask most, and the honest answer is that the prescriber decides it.

Oolite starts the tablet's calendar on the day the prescriber names, applies the morning rules from the first tablet, and draws the tablet's level from the first tablet, since the label gives no level comparison with the pen.

Questions people ask

I am on 1 mg or 2 mg of Ozempic injection. Where do I start on the tablet?

The label's switching rule is written for the 0.5 mg injection only. For higher doses it gives no table, which makes the starting strength a prescriber decision, not something to read off this page.

Why wait a week?

The weekly injection is still delivering for days after the last dose. Starting the tablet the next day would stack the two. One week is the label's margin, the same as for the Wegovy switch.

Do I have to start at 1.5 mg?

Not by the label. The 1.5 mg month is for people starting semaglutide; someone already on the injection goes to 4 mg or 9 mg on the label's rule.

Are the morning rules new to me?

Yes, and they matter. The injection was any time of day, with or without food. The tablet is every morning on an empty stomach, up to 4 ounces of water, then 30 minutes before food, drink or other oral medications.
