Wegovy pill to injection: which dose do I start?

From the 25 mg tablet, the label says to start the 2.4 mg weekly injection the day after the last tablet, or 1.7 mg if 25 mg was not tolerated. With type 2 diabetes, 1.7 mg, then the dose steps. Lower strengths: no rule.

The short answer

From the 25 mg tablet, the label says to start the 2.4 mg weekly injection the day after the last tablet, or 1.7 mg if 25 mg was not tolerated. With type 2 diabetes, 1.7 mg, then the dose steps. Lower strengths: no rule.

What the label says

Section 2.5 of the prescribing information covers both directions. The way from the tablets to the shot reads:

From the label

Patients may switch from WEGOVY® 25 mg tablets to WEGOVY® injection.

From the label

The day after discontinuing WEGOVY® tablets 25 mg once daily, initiate WEGOVY® 2.4 mg subcutaneous injection once weekly. For patients who do not tolerate WEGOVY® 25 mg tablets, consider switching to WEGOVY® 1.7 mg injection.

And a third case, for people with type 2 diabetes:

From the label

If additional weight reduction is needed in patients with type 2 diabetes mellitus treated with WEGOVY® 25 mg tablets, consider switching to WEGOVY® 1.7 mg injection and follow the recommended dosage escalation for WEGOVY® injection.

Section 2.3, on the tablets, points the same way when the top tablet strength does not suit:

From the label

If patients do not tolerate the 25 mg once daily maintenance dosage, consider switching to WEGOVY® injection 1.7 mg once weekly

The three cases, in plain words

On 25 mg, tolerated well. The last tablet one morning, the first 2.4 mg shot the next day, then one shot a week on that day.

On 25 mg, not tolerated. The label suggests the 1.7 mg shot instead. For weight management in adults, the label lists 1.7 mg and 2.4 mg as the two maintenance doses of the injection, with 2.4 mg recommended. Whether you stay at 1.7 mg or move up later is your prescriber's decision.

On 25 mg, with type 2 diabetes. 1.7 mg, then the injection's own dose steps. Section 8.6 explains the context: in people with type 2 diabetes, average semaglutide levels on the 25 mg tablet were lower than on the 2.4 mg injection, and the tablets were not studied for weight reduction in that group.

Where the label is silent

The switching rules start from 25 mg. The label gives no rule for someone on the 1.5, 4 or 9 mg tablet. It does not convert those strengths into a shot dose, and it does not say whether to restart the injection at its first step. That choice belongs to the prescriber, who knows how the steps went for you.

The label also gives no reason for "the day after". What it does give: a tablet reaches its peak level about an hour after the dose, the injection 1 to 3 days after, and both leave the body with the same half-life of about one week. The tablets and the injection are not used together.

What people actually do

On the pill forums, a return to the injection is a recurring story, mostly from people who started on the shot, moved to the tablets, and were disappointed by them after a few months. The detail that decides the label's answer is the tablet strength reached, and it is the first thing worth bringing to the prescriber.

If you are going the other way, from the shot to the tablets, the Wegovy pill page on switching has the label's rule for that direction.

Oolite logs each tablet and each shot with its dose and its date, and the report for your prescriber shows the day the form changed.

Questions people ask

Do I start the shot at 0.25 mg after the tablets?

Not from 25 mg, by the label. It says to start 2.4 mg once weekly the day after the last 25 mg tablet, or to consider 1.7 mg if the 25 mg tablet was not tolerated. The 0.25 mg start is written for people starting Wegovy.

I was on the 4 mg or 9 mg tablet. Which shot do I start?

The label's switching rules are written for the 25 mg tablet only. It gives no rule for 1.5, 4 or 9 mg. Your prescriber chooses the starting dose of the injection.

Can I take my last tablet and my first shot on the same day?

The label says the day after discontinuing the tablets. It also says Wegovy tablets and the injection are not to be used together with other semaglutide products, so the two do not overlap.

Why does the label mention type 2 diabetes separately?

In people with type 2 diabetes, the label says blood levels on the 25 mg tablet were lower on average than on the 2.4 mg injection, and the tablets were not studied for weight reduction in that group. It suggests 1.7 mg, then the injection's dose escalation.
