Foundayo and statins: the simvastatin limit

The label sets one limit: do not go above 20 mg of simvastatin a day with Foundayo, because Foundayo doubled the level of simvastatin's active form in its study. For atorvastatin and rosuvastatin it reports smaller rises and sets no dose limit. Taking the two medicines two hours apart did not change the simvastatin result.

The short answer

The label sets one limit: do not go above 20 mg of simvastatin a day with Foundayo, because Foundayo doubled the level of simvastatin's active form in its study. For atorvastatin and rosuvastatin it reports smaller rises and sets no dose limit. Taking the two medicines two hours apart did not change the simvastatin result.

What the label says

The drug interactions section has a table of medicines whose levels Foundayo changes. Simvastatin is the first line:

From the label

Do not exceed simvastatin 20 mg once daily when concomitantly used with FOUNDAYO.

The reason follows in the same table:

From the label

Use of FOUNDAYO with simvastatin increased exposure of the active metabolite simvastatin acid two-fold

Simvastatin is turned into simvastatin acid in the body, and that is the form that works. In the label's study, Foundayo doubled it. The label adds that a two-fold increase could be clinically meaningful at the highest simvastatin dose, which is why it caps the dose rather than forbidding the combination.

Spacing the two does not help

A natural idea is to take the statin at night and Foundayo in the morning. The pharmacology section answers it: the increase in simvastatin acid was seen whether Foundayo was given at the same time or the two doses were staggered by 2 hours. The limit holds whatever the hour.

Atorvastatin and rosuvastatin

The same pharmacology table measured the two other common statins, with Foundayo at its higher doses:

  • atorvastatin 40 mg: about 1.5 times the usual exposure,
  • rosuvastatin 20 mg: about 1.7 times the usual exposure.

These rises are smaller, and the label does not turn them into a rule: there is no dose limit for atorvastatin or rosuvastatin in the drug interactions section. They are numbers your prescriber can see on the label; they are not a reason to stop or change a statin on your own.

What it means in practice

  • On simvastatin 20 mg a day or less: the label's limit is already met.
  • On simvastatin above 20 mg a day: this is a conversation to have with your prescriber before or at the start of Foundayo. They decide whether to lower the dose or change statin.
  • On atorvastatin or rosuvastatin: no limit on the label. Mention Foundayo to whoever prescribes your statin, as you would any new medicine.

Bring the list of everything you take, statin and dose included, to the appointment where Foundayo is prescribed. The label's other interactions, through the liver enzyme that clears Foundayo, are covered in the answer on other medications.

The semaglutide pills are different

This rule belongs to orforglipron, the molecule in Foundayo. The Wegovy pill's label lists rosuvastatin among the medicines that showed no clinically significant change with semaglutide, and it has no statin limit. When people compare the two pills, this is one of the places where their labels do not say the same thing.

Questions people ask

I take 40 mg of simvastatin. What should I do?

Talk to your prescriber before starting Foundayo, or as soon as possible if you already have. The label says not to exceed 20 mg of simvastatin a day with Foundayo. Changing a statin dose or switching statins is a prescriber's decision, never one to make alone.

Can I take my simvastatin at night and Foundayo in the morning to avoid the problem?

The label studied that. The rise in simvastatin's active form was seen whether the two were taken together or two hours apart. Spacing them does not remove the limit.

Is there a limit for atorvastatin or rosuvastatin?

No. The label reports that Foundayo raised atorvastatin levels about 1.5-fold and rosuvastatin about 1.7-fold in its study, and it sets no dose limit for either. Those numbers are for your prescriber to weigh with the rest of your treatment.

Does the Wegovy pill have the same limit?

No. The Wegovy label lists rosuvastatin among the medicines whose levels did not change in a clinically significant way with semaglutide, and it sets no statin limit. The simvastatin rule is specific to Foundayo.
