Can I take the Wegovy pill with birth control pills?

The label tested it. With semaglutide tablets, it found no clinically significant change in the levels of ethinyl estradiol and levonorgestrel, the estrogen and the progestin of a common combined pill. It has no 30-day backup rule like Foundayo. The morning rule still applies: other tablets wait 30 minutes.

The short answer

The label tested it. With semaglutide tablets, it found no clinically significant change in the levels of ethinyl estradiol and levonorgestrel, the estrogen and the progestin of a common combined pill. It has no 30-day backup rule like Foundayo. The morning rule still applies: other tablets wait 30 minutes.

What the label says

The Wegovy label reports a study made for exactly this question. Researchers gave other oral medicines alongside semaglutide tablets at steady state and measured what happened to them. The result for the hormones of the combined pill is one line in a list:

From the label

No clinically significant differences in the pharmacokinetics of the following drugs were observed when used concomitantly with semaglutide: lisinopril, S-warfarin, R-warfarin, metformin, digoxin, ethinyl estradiol, levonorgestrel, furosemide or rosuvastatin.

Ethinyl estradiol and levonorgestrel are the estrogen and the progestin of a common combined birth control pill. Their levels did not move in a way the label considers meaningful.

Why Foundayo has a rule and the Wegovy pill does not

Foundayo is a different molecule, orforglipron, and its label does ask for something:

From the label

for 30 days after initiation with FOUNDAYO and for 30 days after each dose escalation

During those windows, people on an oral contraceptive are asked to switch to a non-oral method or add a barrier method. Nothing of the kind appears on the Wegovy label. The two tablets are often discussed together, and this is one of the places where their labels really differ. The Foundayo answer explains its rule in full.

What still applies

The label's general caution about oral medicines stays true:

From the label

WEGOVY causes a delay of gastric emptying and thereby has the potential to impact the absorption of concomitantly administered oral medications.

It asks prescribers to monitor the effects of oral medicines taken alongside, with more attention for those that need close monitoring. And it sets the order of the morning: the Wegovy pill first, alone, then at least 30 minutes before food, drinks or other oral medications.

In practice

The birth control pill does not need to move to the evening. It only needs to come after the 30 minutes, with breakfast or later, at the same time each day as usual. If the time of your contraceptive pill matters for its own instructions, your pharmacist can help fit the two together.

What people actually do

On the forums, contraception comes up in the same breath as the other daily medicines: people ask whether the Wegovy pill changes how their other tablets are absorbed. The label's study is the answer for the two hormones it tested, and the 30 minutes is the answer for the order of the morning.

Oolite shows the 30-minute countdown on the lock screen after the morning tablet, so the next pill of the day comes at the right moment.

Questions people ask

Do I need a backup method on the Wegovy pill?

The label does not ask for one. Its interaction study found no clinically significant change in ethinyl estradiol or levonorgestrel with semaglutide tablets. The 30-day backup rule you may have read about is on the Foundayo label, a different molecule.

Can I swallow my birth control pill with the Wegovy pill?

Not at the same moment. The Wegovy pill goes first, alone, with up to 4 ounces of water. The label asks you to wait at least 30 minutes before any other oral medication, your birth control pill included.

What about a progestin-only pill or another hormone?

The study the label reports covers ethinyl estradiol and levonorgestrel. It does not list other contraceptive hormones, so this page cannot speak for them. That is a question for your pharmacist.

Does the label say anything about pregnancy plans?

Yes. It asks to stop at least two months before a planned pregnancy, because semaglutide stays in the body a long time. The pregnancy answer covers that rule.
